Studio Agreement Use this form to agree to the policies and procedures found in the Production Center Studio Manual. This agreement is required to use the Production Center studio space
Equipment Room Agreement Use this form to agree to the policies and procedures found in the Production Center Equipment Room Manual. This agreement is required to set up a check-out account at the Equipment Room.
Greenlight Production Form This form is required for all student productions at Biola University. This form combines the former Greenlight, Production Approval, and Learning Project forms in one easy location. Please be as thorough as possible in your answers to allow Biola to process your request as fast as possible.
Game Design and Interactive Media Greenlight Form This form is only for projects within Game Design and Interactive Media under professor Michael Steffen. Upon approval you will be granted both VPN and Subversion repository access.
Finished Project Submission Use this form to submit your final project for CMA archival.
On-Campus Film Permit This form is required to obtain permission to film on campus. Signatures are required in consecutive order. Film permits should be requested a minimum of one week in advance. A fully approved hard copy must be with the film crew on the days of the shoot and available upon request by Campus Safety.
Studio Reservation Request Use this form to request a studio reservation at the Production Center.
Screening Reservation Request Use this form to request a screening reservation at the Production Center, Rood 55, or Rood 56.
Graphics Lab Reservation Use this form to request access to the Graphics Lab.
Advanced Equipment Training: Freshman Request Use this form to request special permission to check out advanced-level equipment without a training seminar. Equivalent experience and faculty recommendation are required.
Production Safety & Incident Report Form This document includes information about what to do if an emergency or accident occurs on set. It also includes an incident report form to properly document an emergency or accident.

Production Resource Forms

Frame.io Account Request Use this form to request a Frame.io Team account for your production.
CMA File Server Storage Request Use this form to request file storage space for the duration of your production. You can use this space for things like backing up your raw camera footage or storing your editing project files to collaboratively edit from the same media files.

Insurance and Legal Forms

Insurance Certificate Request Use this form to request an insurance certificate from Biola's Risk Management department for third-party gear rentals.
Contract Submission Use this form to request an authorized Biola personnel to sign production-related contracts. Please note that students are under no circumstances allowed to sign on behalf of Biola University.
DMV Motor Vehicle Report (MVR) Authorization Form
Use this form to request a Motor Vehicle Report as part of the process of getting access to drive the Grip Truck. Note this form must be renewed each semester.
Music License
Use this form as a basic music release for requests from music owners.
Location Release Use this form as a basic release for your locations.
Film/Video Consent and Release Use this form for a basic photographic release for your talent.
Production Center Long Distance Equipment Checkout Request Use this form to request equipment checkout approval outside Los Angeles and Orange Counties.
